Appointments

The first step towards a healthy and beautiful smile is to schedule an appointment.   Please contact our office by phone or complete the appointment request form.  Our scheduling coordinator will contact you to confirm your appointment.

If you are unable to keep an appointment, we ask that you kindly provide us with at least 24 hours notice.  We ask for this advanced notice so that we may offer this time to another patient.  

Insurance and Billing

At Family Dental Associates of Warren, we make every effort to provide you with the finest care and convenient financial options.  We are in-network with the following insurance companies:  Aetna, Assurant, Cigna, Delta, Guardian, Horizon BCBS, Metlife, and United Concordia.

If you are covered by another insurance company with dental benefits, and you are able to go out of network, we can verify coverage and will gladly submit all claims for your reimbursement.  If you have any questions, please call us at (732) 560-5988.

We do our best to understand each policy but ultimately it is the patient’s responsibility to know individual coverage.  Our estimates are never a guarantee of payment from an insurance company.  We will always provide you with a copy of our estimates for your own personal records.

Financial Policy

Estimated copayments required for known procedure services are due at the time of service unless other arrangements have been made in advance.  For your convenience we accept American Express, Visa, Master Card and Discover.

Infection Control

Patient safety is our highest priority.  Infection control and universal precautions protect our patients and staff.  Dr. Kandou and Dr. Wang follow procedures and guidelines recommended by governing agencies as the Centers for Disease Control and Prevention (CDC), Occupational Safety & Health Administration (OSHA), and the Environmental Protection Agency (EPA).  These measures include:

•  Hospital-grade disinfection of countertops and treatment surfaces
•  Heat sterilization of equipment before every patient use.
•  Disposable materials in treatment operatories.

We sterilize equipment, including dental handpieces.  Heat sterilization by steam and pressure is achieved by the use of an autoclave to kill bacteria and viruses.  Our dental treatment water is specially monitored to make certain it is safe.
